An opportunity is available for a skilled and compassionate Registered Nurse with experience in home health to provide vital care to patients in a field-based setting. This full-time, 13-week contract role is ideal for professionals who thrive working independently and value patient-centered service.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and create individualized care plans.
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Evaluate patient progress, detect changes in condition, and communicate promptly with the healthcare team and physicians.
• Educate patients and families about disease management, medications, and preventive approaches.
• Document with accuracy and timeliness in Home Care Home Base (HCHB).
• Collaborate with interdisciplinary healthcare professionals to ensure seamless care.
• Participate in case conferences, staff meetings, and ongoing training as required.
• Uphold all regulatory and agency guidelines for care delivery and documentation.

Qualifications:

• Active, unencumbered RN license in state of practice.
• At least 1 year of recent home health nursing experience in a field setting (preferred).
• Proficiency in Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment, communication, and organizational skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to manage a variable caseload and operate autonomously.

Preferred:

• BSN degree.
• Experience carrying a caseload of 2532 points weekly.
• Familiarity with Medicare and home health compliance regulations.

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
