Provide expert nursing care in patients homes as a Home Health Registered Nurse (RN) delivering comprehensive, compassionate care to support recovery and quality of life. This full-time, 13-week contract position offers the opportunity to work independently while collaborating with an interdisciplinary team to meet individualized patient goals.

Key Responsibilities:

• Perform thorough OASIS assessments and develop personalized care plans.
• Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, identifying changes and communicating effectively with physicians and care teams.
• Educate patients and families on treatment plans, medications, and wellness strategies.
• Maintain accurate electronic documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to ensure well-coordinated care.
• Participate in case conferences, staff meetings, and ongoing education.
• Adhere to regulatory and agency standards for compliance and care quality.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum one year recent home health experience; field experience preferred.
• Proficient with Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical assessment capabilities and critical thinking.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to manage a flexible, field-based schedule independently.
• Excellent communication, organizational, and time management skills.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience managing 2532 points weekly caseload.
• Knowledge of Medicare and home health compliance standards.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Caseload averages 2532 points per week (35 patient visits daily).
