An exciting opportunity awaits an experienced and compassionate Registered Nurse to deliver high-quality care as a Home Health RN. Schedule and Assignment Details:

• Contract position: Full-time, 13-week duration
• Hours: Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week, typically 35 patient visits per day
• Field-based, serving patients within a designated local territory
• Electronic documentation: Home Care Home Base (HCHB)
• Assessment tool: OASIS

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop personalized care plans
• Deliver skilled nursing care: wound management, medication administration, IV therapy, and oversight of chronic conditions
• Monitor patients status, document clinical changes, and communicate effectively with physicians and the interdisciplinary care team
• Provide education and resources to patients and families regarding condition management, treatment plans, and medications
• Maintain timely, accurate records in HCHB
• Collaborate with therapists, social workers, and aides to ensure coordinated, holistic care
• Participate in case conferences, staff development, and maintain compliance with regulatory standards

Required Qualifications:

• Active RN license in good standing
• Minimum of 1 year recent home health experience, with strong field proficiency
• Hands-on experience with HCHB and OASIS
• Reliable transportation, valid drivers license, and insurance
• Excellent assessment, communication, and organizational abilities
• Ability to manage a flexible field-based schedule

Preferred Qualifications:

• BSN
• Experience managing a 2532 point caseload
• Familiarity with Medicare and home health compliance

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
