An opportunity is available for an experienced and compassionate Registered Nurse specializing in Home Health to deliver skilled nursing care directly to patients in their homes. This 13-week contract role is ideal for RNs who are organized, self-motivated, and passionate about patient-centered care. You will manage a field-based caseload, using your clinical expertise and excellent communication skills to help patients achieve their health goals while maintaining comfort and dignity at home.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions: wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient conditions, coordinate with medical providers, and keep care teams updated on progress or changes.
• Educate patients and caregivers about care plans, medications, disease processes, and preventive care strategies.
• Document accurately and efficiently using Home Care Home Base (HCHB).
• Collaborate with other healthcare professionals including therapists and home health aides for optimal patient outcomes.
• Participate in staff meetings, case conferences, and ongoing clinical education.
• Consistently uphold regulatory standards in patient care and documentation.
• Travel daily to patient homes within your assigned area; mileage reimbursement is provided.

Qualifications:

• Active RN license in good standing.
• At least 1 year of recent experience in Home Health; field-based background preferred.
• Solid proficiency with Home Care Home Base (HCHB) and OASIS.
• Excellent critical thinking, organization, and communication skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to work independently and manage a flexible schedule.
• BSN and experience managing 2532 points per week strongly preferred.
• Familiarity with Medicare and home health compliance is a plus.

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
