Provide expert nursing care with a focus on patients homes as a skilled Home Health Registered Nurse (RN). This role demands clinical excellence, compassionate communication, and strong organizational skills to deliver personalized healthcare in community settings.

Schedule:
- Full-time, Monday Friday
- 8:00 AM 5:00 PM
Contract Length:
- 13-week assignment
Key Responsibilities:
- Conduct thorough OASIS assessments and develop tailored care plans
- Administer skilled nursing care including wound care, medication management, IV therapy, and chronic disease oversight
- Monitor patient progress, detect changes, and communicate promptly with healthcare teams and physicians
- Educate patients and families on disease processes, treatment protocols, medication management, and preventive care
- Maintain accurate, timely electronic documentation using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary teams including therapists, social workers, and home health aides to ensure coordinated care
- Participate in case conferences and staff development activities
- Comply fully with regulatory and agency standards
Qualifications & Experience:
- Active RN license in good standing in the practicing state
- Minimum one year recent home health nursing experience, preferably field-based
- Proficient with Home Care Home Base (HCHB) and experienced with OASIS assessments
- Strong clinical judgment, critical thinking, and assessment skills
- Reliable transportation with valid drivers license and auto insurance
- Ability to work independently in a flexible, field-based environment
- Excellent communication, time management, and organizational abilities
Preferred:
- Bachelor of Science in Nursing (BSN)
- Experience managing 2532 points per week caseload
- Knowledge of Medicare and home health regulatory compliance
