Experienced and caring registered nurses are invited to provide essential healthcare services in a field-based home health role that prioritizes patient-centered support. This full-time, 13-week contract position offers the opportunity to independently deliver skilled nursing care in patients homes while collaborating within a dedicated healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Primary Responsibilities:

• Conduct comprehensive OASIS assessments and craft individualized care plans.
• Deliver skilled nursing interventions, such as wound care, IV therapy, medication administration, and chronic disease management directly to patients at home.
• Monitor patient progress, promptly identify any changes in condition, and effectively communicate with physicians and interdisciplinary teams.
• Educate patients and family members about diagnoses, home care techniques, medications, and preventive health.
• Complete timely and accurate documentation using Home Care Home Base (HCHB).
• Collaborate regularly with physical therapists, occupational therapists, social workers, and home health aides.
• Participate in case conferences, staff development meetings, and continuing education as needed.
• Maintain compliance with all regulatory, documentation, and agency care standards.

Qualifications:

• Active RN licensure in Maryland, in good standing.
• At least 1 year of recent home health nursing experience (field experience preferred).
• Proficiency with Home Care Home Base (HCHB) and required OASIS experience.
• Demonstrated clinical assessment, organizational, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a field-based caseload (2532 points per week, typically 35 visits daily).
• BSN, experience with Medicare compliance, and managing higher caseloads preferred.
