Deliver compassionate and high-quality care as a Home Health Registered Nurse by supporting patients in the comfort of their own homes. In this impactful role, youll take the lead in comprehensive patient assessments, build individualized care plans, and champion continuity of care through collaboration with a dedicated interdisciplinary team. Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract, full-time
• Field-based, daily travel within assigned territory (mileage reimbursement provided)
• Caseload: 2532 points per week (typically 35 patient visits per day)

Key Qualifications:

• Active RN license in good standing
• At least 1 year of recent Home Health nursing experience (field preferred)
• Thorough proficiency in Home Care Home Base (HCHB) electronic documentation
• Demonstrated experience with OASIS assessments
• Strong clinical assessment and decision-making abilities
• Reliable transportation, valid drivers license, and auto insurance
• Ability to manage a flexible schedule and work independently
• Professional communication, time management, and organizational strengths
• BSN and experience with Medicare/home health compliance preferred

Core Responsibilities:

• Conduct full OASIS assessments and develop targeted, patient-centered care plans
• Deliver a wide scope of skilled nursing interventions such as wound care, medication administration, and chronic disease management
• Monitor patient progress and quickly escalate changes in condition to the healthcare team
• Provide education to patients and families for disease management and prevention
• Maintain accurate and timely records in HCHB
• Collaborate closely with physical and occupational therapists, social workers, and home health aides
• Participate in case conferences, staff meetings, and ongoing education as needed
• Ensure regulatory and agency standards for documentation and care delivery are met
