Make a meaningful difference by delivering compassionate end-of-life care as a Registered Nurse specializing in hospice services. In this contract role, you will play a vital role in supporting patients and their families through the hospice journey, ensuring comfort, dignity, and quality of life in a home or residential setting.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation may be required
- 13-week contract assignment
Key Responsibilities:
- Conduct comprehensive nursing assessments and develop individualized hospice care plans
- Deliver skilled nursing care focused on symptom management, pain control, and patient comfort
- Administer medications and interventions following physician orders and hospice guidelines
- Monitor and communicate changes in patient condition to the care team
- Educate patients and their families on disease progression, comfort strategies, and medication administration
- Collaborate closely with interdisciplinary team members to address holistic patient and family needs
- Document care promptly and accurately using Home Care Home Base (HCHB)
- Participate in case conferences, team meetings, and ongoing quality initiatives
- Maintain full compliance with hospice protocols and regulatory standards
Qualifications:
- Active RN license (in good standing) in the state of practice
- At least 1 year of recent hospice nursing experience
- Proficient in Home Care Home Base (HCHB) documentation
- Excellent communication, organizational, and clinical assessment skills
- Reliable transportation, valid drivers license, and insurance
- Emotional resilience and a compassionate approach to care
- Preferred: BSN and additional palliative care experience; familiarity with Medicare regulations
