A skilled and compassionate Home Health Registered Nurse (RN) is needed for a 13-week contract assignment, providing high-quality nursing care to individuals in their homes. This role emphasizes autonomy, clinical expertise, and collaboration as you help patients achieve their health goals in a supportive environment.

Schedule and Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based work with daily travel to patient residences within an assigned territory
• Caseload: Manage 2532 points per week (typically 35 patient visits per day)
• Utilize Home Care Home Base (HCHB) for electronic documentation and OASIS for assessment

Key Responsibilities:

• Conduct comprehensive nursing assessments and create tailored care plans following OASIS standards
• Deliver skilled nursing interventions, such as wound care, IV therapy, medication administration, and ongoing disease management
• Monitor patient status, promptly report changes, and coordinate with the healthcare team and physicians
• Educate patients and families on medication, care plans, and preventive health measures
• Maintain accurate, timely clinical documentation using HCHB
• Collaborate with therapists, medical social workers, and aides for coordinated patient care
• Attend case conferences, staff meetings, and pursue continuing education as needed
• Ensure compliance with all agency and regulatory guidelines

Required Qualifications:

• Current, unencumbered RN license in the state of practice
• Minimum of one year of recent home health experience, preferably field-based
• Proficient with Home Care Home Base (HCHB) and OASIS
• Excellent assessment, time management, communication, and organizational skills
• Reliable vehicle, valid drivers license, and current auto insurance

Preferred:

• Bachelors degree in Nursing (BSN)
• Previous experience managing a 2532 point weekly caseload
• Familiarity with Medicare compliance standards
