Drive patient care excellence as an experienced Home Health Registered Nurse supporting individuals in a home-based setting. Bring clinical expertise and compassionate leadership to the role of Home Health Clinical Manager, ensuring high standards of care and compliance with regulatory guidelines throughout the duration of this contract position.

Qualifications:

• Active and unrestricted Registered Nurse (RN) license
• Proven experience in Home Health nursing
• Strong clinical management or supervisory background in Home Health preferred
• Exceptional assessment, communication, and organizational skills
• Ability to coordinate multidisciplinary care and manage caseloads independently
• Knowledge of electronic documentation systems and regulatory standards (Medicare, HIPAA, etc.)

Schedule:

• Full-time contract position

Responsibilities:

• Oversee daily clinical operations for a team of Home Health professionals
• Conduct patient assessments, create and review care plans, and ensure effective coordination of services
• Support and mentor RNs and clinical staff in delivery of safe, patient-centered care
• Serve as the primary contact for regulatory compliance, quality improvement, and best practice initiatives
• Participate in case conferencing, staff meetings, and performance evaluations
• Maintain accurate documentation and timely reporting
• Collaborate with physicians, therapists, and family members to achieve optimal patient outcomes

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
