Experienced and compassionate registered nurses seeking a home health setting will find a supportive and dynamic opportunity in this role. This position involves delivering skilled nursing care in patient homes, fostering continuity, outstanding communication, and patient-centered care. The ideal professional is motivated, adaptable, detail-oriented, and comfortable working autonomously while maintaining high clinical and documentation standards.
Schedule and Assignment Details:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Field-based, with daily travel to patient homes in the assigned territory (mileage reimbursement provided)
Primary Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Provide expert nursing care: wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient progress, promptly recognize changes in condition, and communicate with the interdisciplinary care team.
- Educate patients and families on treatment plans, medications, and preventive health strategies.
- Ensure accurate, timely documentation via Home Care Home Base (HCHB).
- Collaborate closely with therapists, social workers, and aides to deliver coordinated, high-quality care.
- Participate in case conferences, staff meetings, and educational activities as required.
- Uphold all regulatory standards and agency policies for documentation and patient care.
Minimum Requirements:
- Active RN license (in good standing) in the state.
- At least 1 year of recent home health nursing experience, preferably field-based.
- Proficiency in Home Care Home Base (HCHB) and OASIS required.
- Strong clinical judgment, organizational, and communication skills.
- Valid drivers license, reliable transportation, and current auto insurance.
Preferred:
- BSN degree.
- Prior experience managing a caseload of 2532 points weekly.
- Familiarity with Medicare and home health compliance.
