Experienced and compassionate registered nurses seeking a home health setting will find a supportive and dynamic opportunity in this role. This position involves delivering skilled nursing care in patient homes, fostering continuity, outstanding communication, and patient-centered care. Schedule and Assignment Details:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Field-based, with daily travel to patient homes in the assigned territory (mileage reimbursement provided)

Primary Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Provide expert nursing care: wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, promptly recognize changes in condition, and communicate with the interdisciplinary care team.
• Educate patients and families on treatment plans, medications, and preventive health strategies.
• Ensure accurate, timely documentation via Home Care Home Base (HCHB).
• Collaborate closely with therapists, social workers, and aides to deliver coordinated, high-quality care.
• Participate in case conferences, staff meetings, and educational activities as required.
• Uphold all regulatory standards and agency policies for documentation and patient care.

Minimum Requirements:

• Active RN license (in good standing) in the state.
• At least 1 year of recent home health nursing experience, preferably field-based.
• Proficiency in Home Care Home Base (HCHB) and OASIS required.
• Strong clinical judgment, organizational, and communication skills.
• Valid drivers license, reliable transportation, and current auto insurance.

Preferred:

• BSN degree.
• Prior experience managing a caseload of 2532 points weekly.
• Familiarity with Medicare and home health compliance.
