Job Description
Deliver skilled, patient-centered nursing in a home health setting, making a meaningful difference for individuals in need of compassionate care and clinical expertise. This contract assignment offers the opportunity to work independently within a supportive interdisciplinary team dedicated to high-quality outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Daily field-based travel to patient residences (mileage reimbursement provided)
- Caseload of 2532 points per week (typically 35 patient visits per day)
Responsibilities:
- Complete comprehensive OASIS assessments; develop and manage individualized care plans
- Deliver skilled nursing interventions: wound care, medication administration, IV therapy, and chronic disease management
- Monitor and document patient progress, recognizing and reporting changes promptly to the healthcare team and physicians
- Educate patients and families on diagnoses, care plans, medications, and preventive health practices
- Document thoroughly and accurately in Home Care Home Base (HCHB)
- Collaborate with therapists, aides, and social workers to ensure holistic, coordinated care
- Participate actively in case conferences, staff meetings, and continuing education opportunities
- Maintain compliance with regulatory, agency, and documentation standards
Qualifications:
- Current RN licensure (in good standing) in the state of practice
- Minimum 1 year recent Home Health RN experience (field experience preferred)
- Proficient with Home Care Home Base (HCHB) and OASIS assessments
- Excellent assessment, communication, and organizational skills
- Valid drivers license, reliable vehicle, and insurance required
- Ability to work effectively, independently, and flexibly in the field
- BSN and prior experience managing a caseload of 2532 points weekly preferred
- Knowledge of Medicare and home health regulations a plus
