Job Description

An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care to patients within their homes. This role emphasizes quality, continuity, and patient-centered care while operating with a high level of clinical expertise and independent judgment.

Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans
  • Provide skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management
  • Evaluate patient progress and communicate any changes promptly to the care team and physicians
  • Educate patients and families on disease management, treatment plans, medications, and preventive care
  • Maintain accurate documentation in Home Care Home Base (HCHB)
  • Collaborate with therapists, social workers, and home health aides to ensure coordinated care
  • Participate in case conferences, staff meetings, and continuing education
  • Comply with regulatory and agency standards for documentation and patient care

Qualifications:

  • Active RN license in good standing within the state of practice
  • Minimum 1 year recent home health experience, preferably field-based
  • Proficient in Home Care Home Base (HCHB) and OASIS assessment tools
  • Strong clinical assessment and critical thinking skills
  • Reliable transportation with valid drivers license and auto insurance
  • Able to work independently and manage a flexible, field-based schedule
  • Excellent communication, time management, and organizational abilities

Preferred:

  • BSN degree
  • Experience managing 2532 points per week caseload
  • Familiarity with Medicare and home health compliance standards

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points weekly (~35 patient visits per day)
  • Field-based with daily travel to patient homes within assigned territory (mileage reimbursement provided)
