As a Home Health Registered Nurse (RN), youll deliver high-quality nursing care to individuals in their own homes, supporting patient independence, safety, and comfort. This is a 13-week, full-time contract with a schedule of Monday through Friday, 8:00 AM 5:00 PM. Each week, youll manage a caseload of 2532 points (approximately 35 patient visits per day), utilizing the Home Care Home Base (HCHB) electronic documentation system, and performing OASIS assessments.
Key Responsibilities:
- Perform comprehensive OASIS assessments and build patient-specific care plans
- Deliver skilled nursing interventionswound care, medication administration, IV therapy, and chronic disease management
- Track patient progress accurately and communicate any status changes promptly to physicians or care teams
- Educate patients and families about disease management, treatment plans, and medication regimens
- Maintain timely and precise documentation within HCHB
- Work directly with an interdisciplinary team to ensure comprehensive care, attending case conferences and team meetings as needed
- Adhere to regulatory and agency standards for high-quality home healthcare delivery
- Daily travel to patient homes within the assigned territory (mileage reimbursement provided)
Qualifications:
- Active, unencumbered RN license in the state of practice
- At least one year of recent home health field experience
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools
- Sound clinical assessment and time-management skills
- Dependable transportation, drivers license, and auto insurance
- BSN and experience managing higher caseload points are preferred
