Job Description
Seeking a dedicated and skilled Registered Nurse to deliver high-quality care to patients in a home health setting. This role centers on providing patient-focused, skilled nursing services in the community, managing a diverse caseload, and ensuring thorough and accurate documentation.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- 13-week contract
Primary Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized patient care plans.
- Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and evaluate patient progress, reporting changes in condition promptly to physicians and the interdisciplinary care team.
- Educate patients and families on disease management, medication regimens, and preventive healthcare.
- Document patient care accurately and promptly using Home Care Home Base (HCHB).
- Work collaboratively with therapists, social workers, and home health aides to ensure cohesive, coordinated care.
- Attend case conferences, staff meetings, and engage in ongoing professional education.
- Travel daily to patient homes in the assigned area; mileage reimbursement provided.
Required Qualifications:
- Active RN license (in good standing) in the state of practice.
- At least 1 year of recent home health field experience.
- Proficiency and experience with Home Care Home Base (HCHB) and OASIS documentation.
- Reliable transportation, valid drivers license, and current auto insurance.
- Ability to work independently, prioritize caseload, and maintain strong organizational skills.
- Excellent communication and critical thinking abilities.
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN).
- Experience managing a caseload of 2532 points weekly.
- Familiarity with Medicare and home health compliance.
