An opportunity is available for an experienced and compassionate Registered Nurse to provide exemplary care in a home health setting. This contract position focuses on delivering skilled nursing support to patients in their homes while fostering independence, dignity, and quality outcomes. Success in this role requires strong clinical expertise, excellent communication, and proficiency working independently in a field-based environment.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
- Monitor changes in patient condition and communicate findings to the care team and physicians
- Educate patients and families about disease management, medications, and preventive strategies
- Maintain accurate, timely documentation within Home Care Home Base (HCHB)
- Collaborate closely with physical and occupational therapists, medical social workers, and home health aides
- Attend case conferences, participate in staff meetings, and engage in ongoing professional development
- Consistently adhere to all regulatory and agency requirements regarding documentation and patient care
Preferred Experience and Qualifications:
- Active RN license (in good standing)
- At least 1 year of recent home health nursing experience; direct field experience highly desirable
- Proficient with OASIS and Home Care Home Base (HCHB)
- BSN strongly preferred
- Proven experience managing a caseload of 2532 points per week
- Familiarity with Medicare and home health compliance standards
- Reliable transportation, valid drivers license, and auto insurance
