An opportunity is available for a Home Health Registered Nurse to deliver skilled, patient-focused care in a field-based setting. This role supports patients with chronic and acute care needs, utilizing the latest standards and technology, and offers autonomy and professional growth.

Schedule:

• Full-time, contract for 13 weeks
• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and craft individualized care plans
• Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, recognize changes in condition, and communicate with physicians and care teams
• Educate patients and families on diagnoses, plans of care, medications, and preventive measures
• Accurately document all patient interactions and clinical data in Home Care Home Base (HCHB)
• Collaborate with interdisciplinary teams, including therapists and home health aides, to facilitate coordinated care
• Participate in case conferences and professional development activities as required
• Ensure regulatory compliance in all aspects of care and documentation

Required Qualifications:

• Active RN license in good standing
• At least 1 year of recent, hands-on home health nursing experience
• Proficiency in Home Care Home Base (HCHB) and OASIS assessments
• Sound clinical assessment and decision-making abilities
• Reliable transportation, drivers license, and active auto insurance
• Strong communication, time management, and organizational skills
• Self-motivated and comfortable working independently in the field

Preferred Qualifications:

• BSN degree
• Experience managing a caseload of 2532 points per week
• Familiarity with Medicare and home health compliance guidelines

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
