Job Description

Step into an impactful nursing position providing advanced care to patients in their own homes. As a Home Health Registered Nurse (RN), youll have the opportunity to deliver high-quality, patient-centered care, building lasting relationships while working independently in the field. This full-time, 13-week contract role offers meaningful work with a MondayFriday, 8:00 AM5:00 PM schedule, serving a caseload of approximately 2532 points per week (typically 35 patient visits daily).

Key Responsibilities:

• Complete comprehensive OASIS assessments and establish individualized care plans.
• Deliver skilled nursing services, such as wound care, IV therapy, medication management, and chronic disease support.
• Monitor patient progress, update documentation in Home Care Home Base (HCHB), and communicate changes to the care team and physicians.
• Educate patients and their families on conditions, medications, and preventive care measures.
• Coordinate care with physical therapists, occupational therapists, home health aides, and social workers.
• Attend case conferences, participate in staff meetings, and uphold regulatory and agency care standards.

Required Qualifications:

• Active, unencumbered RN license in the state of practice.
• At least 1 year of recent home health experience in the field.
• Experience using Home Care Home Base (HCHB) and OASIS assessment tools.
• Reliable transportation with a valid drivers license and auto insurance.
• Exceptional assessment, communication, and organizational skills.
• Ability to work independently and manage a field-based, flexible schedule.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience with a 2532-point weekly caseload.
• Familiarity with Medicare regulations and home health compliance.
