Deliver high-quality, person-centered nursing care in a home health setting as a Registered Nurse. This role involves supporting patients in their homes, utilizing your clinical expertise to ensure exceptional outcomes, and collaborating with interdisciplinary healthcare professionals. The full-time, 13-week contract offers weekday hours and a predictable schedule.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based, providing care in patient homes
• Caseload: 2532 points per week (typically 35 daily visits)
• Mileage reimbursement for travel within your assigned territory

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans tailored to each patients needs
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, identify changes in condition, and report to the care team promptly
• Educate patients and families about disease management, medications, and preventive care
• Maintain precise and timely documentation using Home Care Home Base (HCHB)
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides
• Attend case conferences, staff meetings, and educational sessions as needed
• Ensure compliance with all regulatory guidelines and company standards

Required Qualifications:

• Active RN license (in good standing)
• At least 1 year of recent home health experience (field experience preferred)
• Proficiency in OASIS and Home Care Home Base (HCHB)
• Strong clinical assessment, critical thinking, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to self-manage and organize field-based workflow

Preferred:

• BSN degree
• Experience managing 2532 points per week
• Familiarity with Medicare and home health compliance
