An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide skilled nursing care within patient homes. This full-time, 13-week contract position focuses on delivering quality, patient-centered care through comprehensive assessments, individualized care plans, and close collaboration with interdisciplinary teams. The role requires a proactive and organized nurse comfortable working independently while maintaining clinical excellence and strong patient communication.

Location: Baltimore, MD

Schedule:

• Monday Friday
• 8:00 AM 5:00 PM

Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Provide skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify condition changes, and communicate effectively with care teams and physicians.
• Educate patients and families on disease management, treatment plans, and preventive care.
• Maintain timely and accurate documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides for coordinated care delivery.
• Participate in case conferences, staff meetings, and ongoing education.
• Comply with regulatory and agency standards for documentation and patient care.

Qualifications:

• Active RN license in Maryland, in good standing
• Minimum 1 year of recent home health experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation tools
• Strong clinical assessment, critical thinking, and communication skills
• Reliable transportation with valid drivers license and auto insurance
• Ability to manage a flexible, independent, field-based schedule

Preferred:

• BSN degree preferred
• Experience managing 2532 patient points per week
• Knowledge of Medicare and home health compliance standards
