An exciting opportunity awaits for an experienced and compassionate Home Health Registered Nurse (RN) seeking a rewarding contract role. This position involves providing high-quality, patient-centered care in a home health setting, focusing on improving patient outcomes while fostering independence and comfort.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care: medication administration, wound care, IV therapy, and chronic disease management.
• Monitor and document patient progress in Home Care Home Base (HCHB); communicate promptly with physicians and care teams.
• Educate patients and families on disease management, treatment adherence, and preventive health.
• Ensure clear and timely documentation in accordance with agency standards.
• Collaborate with interdisciplinary team members to promote coordinated and holistic care.
• Participate in case conferences, staff meetings, and continuing education activities.
• Adhere to regulatory and agency standards for patient care and documentation.
• Travel daily to patient homes within your assigned territory (mileage reimbursement provided).

Required Qualifications:

• Active and unencumbered RN license for the state of practice.
• Minimum of one year recent home health nursing experience (field-based experience preferred).
• Strong proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Excellent clinical judgment, communication, and organizational skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Self-motivated, able to work independently, and schedule field visits effectively.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Previous management of a caseload of 2532 points per week.
• Familiarity with Medicare guidelines and home health compliance standards.
