Job Description

Providing compassionate end-of-life care requires a skilled nursing professional who combines clinical expertise with empathy and excellent communication. This role focuses on delivering comprehensive hospice services to patients in home and residential settings throughout the Baltimore area. The position is field-based, involving daily travel to patient homes, assisted living facilities, or nursing homes, with mileage reimbursement offered.

Key Responsibilities:

  • Conduct thorough nursing assessments and develop personalized hospice care plans.
  • Deliver skilled nursing care emphasizing symptom relief, pain management, and patient comfort.
  • Administer medications and treatments as prescribed, adhering to hospice protocols.
  • Monitor patient conditions closely and communicate changes to the medical director and care team.
  • Educate patients, families, and caregivers on disease progression, comfort strategies, and medication use.
  • Collaborate with social workers, chaplains, aides, and volunteers to meet patients physical, emotional, and spiritual needs.
  • Complete accurate and timely documentation using Home Care Home Base (HCHB).
  • Participate in case conferences and quality improvement initiatives.
  • Comply with hospice policies and regulatory standards.

Qualifications:

  • Active RN license in good standing within the state of practice.
  • Minimum of 1 year recent hospice nursing experience.
  • Proficiency with Home Care Home Base (HCHB).
  • Strong clinical assessment and communication skills.
  • Reliable transportation, valid drivers license, and insurance.
  • Ability to manage an independent field-based schedule with emotional resilience and compassion. Preferred qualifications include a BSN and familiarity with Medicare hospice regulations.

Schedule:

  • Full-time, 13-week contract.
  • Monday to Friday, 8:00 AM 5:00 PM, with occasional weekend or on-call rotations.
  • Approximately 45 patient visits daily.
