An exciting opportunity is available for a skilled and compassionate Registered Nurse specializing in home health care. In this contract role, you will play a crucial part in delivering high-quality nursing services to patients within the comfort of their homes, ensuring all care remains patient-focused and aligned with physician directives.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Role Overview:
- Provide care as part of a 13-week contract
- Daily travel to patient homes within your assigned territory (mileage reimbursement provided)
- Caseload expectation of 2532 points weekly, typically 35 patient visits each day
- Utilize Home Care Home Base (HCHB) for all electronic documentation
- Conduct patient assessments with OASIS, creating and monitoring personalized care plans
Key Responsibilities:
- Complete detailed OASIS assessments and develop tailored patient care plans
- Deliver skilled nursing interventions: wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, recognize and report clinical changes, and communicate with interdisciplinary care teams
- Educate patients and families regarding disease processes, medications, and care strategies
- Maintain timely and accurate records in compliance with all regulatory guidelines
- Participate in care conferences, team meetings, and ongoing professional development
Qualifications:
- Active, unrestricted RN license
- At least 1 year of recent home health RN experience (field-based strongly preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Strong clinical skills and knowledge of home health compliance standards
- Reliable transportation, drivers license, and auto insurance
- Excellent communication, problem-solving, and time management abilities
- BSN and experience managing a caseload of 2532 points weekly preferred
