A rewarding opportunity awaits an experienced and compassionate Home Health Registered Nurse (RN) ready to provide top-notch clinical care to patients within their homes. This full-time, 13-week contract role is ideal for an autonomous, organized RN with excellent assessment skills and a passion for positively impacting patients lives through direct care and education.
Schedule & Setting
- Monday through Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points weekly (typically 35 patient visits daily)
- Field-based: Delivering care in patient residences
- Daily travel within assigned territory (mileage reimbursement provided)
Responsibilities
- Complete thorough OASIS assessments and develop individualized care plans
- Deliver skilled nursing services: wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress and communicate changes to care teams and physicians
- Educate patients and families on health conditions, medications, and preventive care
- Accurately document visits and care in Home Care Home Base (HCHB)
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides to ensure coordinated care
- Join case conferences, attend staff meetings, and participate in ongoing education
- Uphold all regulatory and agency standards
Requirements
- Active RN license in good standing in the state of practice
- At least 1 year of recent Home Health field experience
- Proficiency with Home Care Home Base (HCHB)
- Experience with OASIS assessments
- Strong clinical, communication, and critical thinking abilities
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently with strong organization and time management skills
- BSN preferred
- Prior experience managing 2532 points/week and knowledge of Medicare home health compliance standards is a plus
