Deliver high-quality care where it matters mostat home. We are seeking an experienced and compassionate Registered Nurse specializing in Home Health to provide skilled, patient-centered nursing care. This role focuses on empowering individuals to achieve optimal wellness in the comfort of their own homes, promoting better outcomes while maintaining a high standard of clinical excellence.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Engage in meaningful, autonomous nursing practice with a manageable caseload (2532 points per week, averaging 35 visits daily) during a 13-week contract. Field-based, this role involves daily travel to patient residences within a defined territory, with mileage reimbursement provided.
Key Qualifications:
- Active RN license (in good standing)
- Minimum 1 year recent experience in Home Health (field experience preferred)
- OASIS and Home Care Home Base (HCHB) proficiency required
- Strong clinical assessment skills and critical thinking ability
- Excellent communication, organization, and time management skills
- Reliable transportation, current drivers license, and auto insurance
Preferred:
- BSN degree
- Prior experience managing caseloads of 2532 points per week
- Familiarity with Medicare and home health compliance
Key Responsibilities:
- Conduct thorough OASIS assessments and tailor care plans to each patient
- Provide skilled nursing care in areas such as wound care, IV therapy, and medication administration
- Monitor patient progress, communicating changes promptly to healthcare teams and physicians
- Educate patients and families on chronic disease management and preventive care
- Maintain timely, accurate clinical documentation in HCHB
- Collaborate with interdisciplinary teams for coordinated patient care
