Job Description

Deliver skilled and compassionate nursing care as a Home Health Registered Nurse, supporting patients in their homes with a focus on quality and continuity. This contract opportunity calls for a self-motivated RN adept at managing clinical care independently, communicating effectively, and maintaining strong patient relationships.

Schedule & Setting

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Field-based position daily travel within an assigned territory
• Electronic documentation via Home Care Home Base (HCHB)
• Mileage reimbursement available

Key Responsibilities

• Complete OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing services such as wound care, medication administration, IV management, and chronic disease monitoring
• Assess patient progress, identify changes, and promptly update care teams and physicians
• Provide education to patients and families regarding diagnoses, treatment plans, and preventive strategies
• Ensure timely, accurate documentation using HCHB
• Collaborate with other clinicians including therapists, social workers, and home health aides
• Participate in case conferences and meetings as needed while maintaining regulatory compliance

Qualifications

• Active RN license in good standing (state-specific)
• At least one year of recent field Home Health RN experience
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment required
• Strong clinical assessment, critical thinking, organizational, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and effectively manage a flexible field schedule

Preferred

• BSN degree
• Previous experience managing 2532 points/week
• Familiarity with Medicare and home health compliance
