Job Description
An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care in patient homes, ensuring seamless, patient-focused service. This contract role is ideal for self-driven nurses who thrive in independent, field-based environments and demonstrate a strong commitment to clinical excellence and clear patient communication.
Schedule and Assignment Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Field-based with daily travel within assigned territory (mileage reimbursement included)
- Electronic documentation using Home Care Home Base (HCHB)
- OASIS assessment tool
Key Responsibilities:
- Conduct OASIS assessments and develop personalized care plans
- Deliver skilled nursing care (wound care, medication administration, IV therapy, disease management)
- Evaluate patient status, communicate changes promptly to healthcare teams and physicians
- Educate patients and families on medication, disease processes, and preventive care
- Complete timely, accurate documentation in HCHB
- Collaborate closely with interdisciplinary team members
- Attend case conferences, staff meetings, and training as required
- Adhere to regulatory and agency documentation and care standards
Qualifications:
- Active RN license (in good standing)
- At least one year of recent home health (field) RN experience
- Proficient in Home Care Home Base (HCHB)
- OASIS assessment experience required
- Strong clinical, assessment, and critical thinking skills
- Valid drivers license, reliable transportation, and auto insurance
- Excellent organizational and communication abilities
- Ability to work autonomously in the field
Preferred:
- BSN
- Previous experience managing 2532 points/week
- Knowledge of Medicare and home health compliance
