Job Description

Deliver vital care directly to patients by taking on the rewarding responsibilities of a Home Health Registered Nurse. This field-based position allows you to form meaningful connections while delivering patient-centered care right in the comfort of their homes. The contract spans 13 weeks with a full-time schedule, ideal for professionals who thrive in dynamic, autonomous environments.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Complete thorough OASIS assessments and develop individualized, goal-oriented care plans.
  • Deliver expert nursing services, such as wound care, medication management, IV administration, and chronic disease support.
  • Routinely evaluate patient status and relay changes promptly to the interdisciplinary healthcare team.
  • Educate patients and caregivers about diagnoses, treatments, medications, and preventative strategies.
  • Ensure all documentation is accurate and timely in Home Care Home Base (HCHB).
  • Collaborate with a broad care teamincluding therapists and aidesto coordinate holistic care.
  • Attend case conferences and remain current with continuing education.
  • Adhere to state and agency policy standards for safe and effective patient care.

Qualifications:

  • Active RN license in good standing within the state.
  • At least one year of recent home health nursing experience in the field.
  • Proficient in Home Care Home Base (HCHB) and experienced with OASIS.
  • Demonstrated clinical assessment, organizational, and critical thinking abilities.
  • Valid drivers license, auto insurance, and dependable transportation.
  • Comfortable managing a flexible, independent field schedule.
  • Excellent interpersonal, communication, and time-management skills.
  • BSN and experience with 2532 points per week preferred.
  • Knowledge of Medicare and home health compliance standards is a plus.

$1,954 - $2,067/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
