Job Description

Passionate about delivering patient-centered care outside of the traditional hospital setting? An opportunity is available for a skilled and compassionate Home Health Registered Nurse (RN) with expertise in home-based care. In this position, you will provide nursing services to patients in their homes, offering both clinical excellence and valuable education to patients and families.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract

Responsibilities:

  • Complete OASIS assessments and establish individualized patient care plans.
  • Deliver skilled nursing care such as wound care, IV therapy, medication administration, and chronic disease management in patient homes.
  • Monitor patient progress, detect changes in condition, and communicate promptly with physicians and the healthcare team.
  • Educate patients and family members regarding conditions, medications, and ongoing care plans.
  • Maintain accurate, timely electronic documentation using the Home Care Home Base (HCHB) system.
  • Work collaboratively with a multidisciplinary care team, including physical therapists, occupational therapists, and home health aides.
  • Participate in case conferences, team meetings, and ongoing professional development activities.
  • Adhere strictly to all agency and regulatory guidelines for high-quality patient care.

Qualifications:

  • Active, unencumbered RN License (state of practice)
  • At least 1 year of recent experience in Home Health; field-based experience highly preferred
  • Proficiency with Home Care Home Base (HCHB) and OASIS required
  • Strong ability for clinical assessment, independent decision-making, and time management
  • Valid drivers license, reliable transportation, and auto insurance
  • BSN preferred; experience handling 2532 patient points per week preferred
  • Familiarity with Medicare and home health compliance standards a plus
