Job Description
Deliver exceptional patient-centered care as a Home Health Registered Nurse, supporting individuals in their homes and making a direct impact on their wellbeing. This full-time, 13-week assignment is ideal for an experienced, compassionate RN who thrives on autonomy, skilled clinical practice, and dynamic interaction with patients and interdisciplinary teams.
Schedule/Hours:
- Monday to Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing care, such as wound management, medication administration, IV therapy, and chronic disease monitoring.
- Track patient progress, recognize and report changes, and collaborate closely with physicians and care teams.
- Provide ongoing education to patients and families on diseases, medications, and self-care strategies.
- Accurately maintain records and documentation using Home Care Home Base (HCHB).
- Partner with physical therapists, occupational therapists, medical social workers, and home health aides to coordinate services.
- Participate in case conferences, team meetings, and professional development activities.
- Uphold all compliance and clinical standards for home health care delivery.
Required Qualifications:
- Active RN license (state of practice), in good standing
- Minimum one year of recent home health nursing experience (field experience highly valued)
- Proficiency in Home Care Home Base (HCHB) and OASIS
- Strong assessment, critical thinking, and clinical decision-making skills
- Reliable transportation, valid drivers license, and current auto insurance
- Excellent communication, time management, and organizational skills
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN)
- Experience managing 2532 points per week
- Knowledge of Medicare and home health compliance
