Experienced and compassionate nurses are invited to bring their expertise to a dynamic home health setting as a key member of a dedicated patient care team. In this role, you will deliver skilled nursing care directly to patients in their homes, with a focus on quality, continuity, and patient-centered outcomes.
Schedule & Hours:
- Monday Friday, 8:00 AM 5:00 PM
Role Overview:
- 13-week full-time contract
- Field-based, direct care (35 patient visits daily; 2532 points/week)
- Utilize Home Care Home Base (HCHB) for electronic documentation
- Use OASIS for comprehensive assessments
- Daily travel within assigned territory (mileage reimbursement included)
Responsibilities:
- Perform OASIS assessments and design individualized care plans
- Administer skilled nursing care (wound care, medication, IV therapy, chronic disease management)
- Monitor patient progress and communicate effectively with the care team and physicians
- Provide education to patients and caregivers regarding disease management and preventive care
- Document care accurately and promptly in HCHB
- Collaborate with the interdisciplinary care team, including therapists and home health aides
- Participate in case conferences and staff meetings
- Adhere to regulatory and agency standards for documentation and patient care
Qualifications:
- Active RN license (state of practice)
- Minimum 1 year of recent home health RN experience in the field
- Home Care Home Base (HCHB) proficiency
- OASIS experience required
- Strong clinical assessment and organizational skills
- Valid drivers license, reliable transportation, and auto insurance
- Ability to work independently and manage a flexible schedule
Preferred:
- BSN
- Prior experience with a 2532 point caseload
- Familiarity with Medicare/home health compliance
