Job Description

Experienced and compassionate nurses are invited to bring their expertise to a dynamic home health setting as a key member of a dedicated patient care team. In this role, you will deliver skilled nursing care directly to patients in their homes, with a focus on quality, continuity, and patient-centered outcomes.

Schedule & Hours:

  • Monday Friday, 8:00 AM 5:00 PM

Role Overview:

  • 13-week full-time contract
  • Field-based, direct care (35 patient visits daily; 2532 points/week)
  • Utilize Home Care Home Base (HCHB) for electronic documentation
  • Use OASIS for comprehensive assessments
  • Daily travel within assigned territory (mileage reimbursement included)

Responsibilities:

  • Perform OASIS assessments and design individualized care plans
  • Administer skilled nursing care (wound care, medication, IV therapy, chronic disease management)
  • Monitor patient progress and communicate effectively with the care team and physicians
  • Provide education to patients and caregivers regarding disease management and preventive care
  • Document care accurately and promptly in HCHB
  • Collaborate with the interdisciplinary care team, including therapists and home health aides
  • Participate in case conferences and staff meetings
  • Adhere to regulatory and agency standards for documentation and patient care

Qualifications:

  • Active RN license (state of practice)
  • Minimum 1 year of recent home health RN experience in the field
  • Home Care Home Base (HCHB) proficiency
  • OASIS experience required
  • Strong clinical assessment and organizational skills
  • Valid drivers license, reliable transportation, and auto insurance
  • Ability to work independently and manage a flexible schedule

Preferred:

  • BSN
  • Prior experience with a 2532 point caseload
  • Familiarity with Medicare/home health compliance
