Job Description
Provide expert nursing care in patient homes as a Home Health Registered Nurse (RN) in the Auburn, KY area. This full-time, 13-week contract position offers a dynamic, field-based role where clinical excellence and patient-centered care are paramount. Manage a caseload of 2532 points weekly, equating to approximately 35 patient visits per day, Monday through Friday from 8:00 AM to 5:00 PM.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, recognize condition changes, and communicate effectively with physicians and the interdisciplinary care team.
- Educate patients and their families about disease processes, treatment regimens, medications, and preventive strategies.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB) electronic system.
- Coordinate care with therapists, social workers, and home health aides.
- Participate in case conferences, staff meetings, and ongoing education.
- Ensure compliance with all regulatory and agency standards.
Qualifications and Experience:
- Active RN license in good standing within the state.
- Minimum of 1 year recent Home Health experience; field experience preferred.
- Proficiency in Home Care Home Base (HCHB) and experience with OASIS assessments required.
- Strong clinical assessment, critical thinking, and communication skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently with excellent time management and organization.
- BSN preferred.
- Familiarity with Medicare and home health compliance standards is a plus.
