Provide expert nursing care in patient homes as a Home Health Registered Nurse (RN) in the Auburn, KY area. This full-time, 13-week contract position offers a dynamic, field-based role where clinical excellence and patient-centered care are paramount. Manage a caseload of 2532 points weekly, equating to approximately 35 patient visits per day, Monday through Friday from 8:00 AM to 5:00 PM.

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize condition changes, and communicate effectively with physicians and the interdisciplinary care team.
• Educate patients and their families about disease processes, treatment regimens, medications, and preventive strategies.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB) electronic system.
• Coordinate care with therapists, social workers, and home health aides.
• Participate in case conferences, staff meetings, and ongoing education.
• Ensure compliance with all regulatory and agency standards.

Qualifications and Experience:

• Active RN license in good standing within the state.
• Minimum of 1 year recent Home Health experience; field experience preferred.
• Proficiency in Home Care Home Base (HCHB) and experience with OASIS assessments required.
• Strong clinical assessment, critical thinking, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently with excellent time management and organization.
• BSN preferred.
• Familiarity with Medicare and home health compliance standards is a plus.
