Job Description

Deliver compassionate end-of-life care as a dedicated Hospice Registered Nurse, supporting patients and families throughout their hospice journey. This field-based contract role emphasizes dignity, comfort, and quality of life in home or residential settings, requiring both clinical excellence and deep empathy. You will collaborate as part of an interdisciplinary teamincluding social workers, chaplains, and nursing assistantsto provide holistic, patient-centered support.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional weekend or on-call rotation may be required

Essential Qualifications:

  • Current, active RN license in good standing
  • At least 1 year of recent hospice nursing experience
  • Proficiency in Home Care Home Base (HCHB) electronic documentation
  • Strong clinical assessment, pain management, and communication skills
  • Reliable transportation, valid drivers license, and proof of auto insurance
  • Ability to manage a field-based caseload independently
  • Compassion, emotional resilience, and an advocacy mindset

Preferred Experience:

  • Bachelor of Science in Nursing (BSN)
  • Background in palliative or end-of-life care
  • Familiarity with Medicare hospice regulations and documentation

Core Responsibilities:

  • Conduct comprehensive assessments and develop individualized care plans
  • Provide skilled nursing interventions aimed at symptom management and patient comfort
  • Administer medications and collaborate closely with clinicians, aides, and volunteers
  • Educate and support patients, caregivers, and family members
  • Participate in case conferences, team meetings, and uphold quality standards
  • Maintain timely and accurate documentation in HCHB
  • Travel daily to patient residences within the assigned area (with mileage reimbursement)
