Exceptional opportunity for a skilled Home Health Registered Nurse (RN) to provide compassionate, patient-centered care in a field-based setting. This full-time, 13-week contract position involves delivering high-quality nursing services to patients in their homes throughout the Atlanta, GA, area. The role emphasizes clinical excellence, patient education, and team collaboration to promote optimal health outcomes.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease care.
- Monitor patient progress, identify changes in condition, and communicate promptly with the healthcare team.
- Educate patients and families on treatment plans, medications, and preventive health measures.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB) software.
- Coordinate care with therapists, social workers, and home health aides.
- Participate in case conferences, staff meetings, and ongoing education.
- Comply with all regulatory and agency standards for documentation and patient care.
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum one year of recent home health experience, preferably field-based.
- Proficiency in Home Care Home Base (HCHB) and OASIS assessment tools.
- Strong clinical assessment, critical thinking, and communication skills.
- Reliable transportation, valid driver's license, and auto insurance.
- Ability to manage a flexible schedule independently.
- BSN preferred.
- Familiarity with Medicare and home health compliance standards is a plus.
- Experience managing 2532 points per week preferred.
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM
- Caseload of 2532 points weekly (35 patient visits daily)
- Daily travel within assigned territory with mileage reimbursement
