Qualifications:
- Current and unrestricted RN license
- Experience providing home health care as a registered nurse
- Proficient in managing patient assessments, developing care plans, and documenting progress
- Strong ability to educate patients and families on care procedures
- Excellent organizational, clinical, and communication skills
- Reliable transportation and a valid drivers license
Responsibilities:
- Conduct in-home patient assessments and ongoing monitoring
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed
