Job Description
Provide skilled nursing care in patients homes as a Home Health Registered Nurse (RN), focusing on delivering quality, patient-centered services while working independently in the field. This full-time, 13-week contract role involves managing comprehensive assessments and individualized care plans to promote optimal health outcomes.
Location: Athens, GA
Schedule:
- Monday Friday
- 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop care plans tailored to individual patient needs.
- Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease care.
- Monitor patient progress, detect changes in condition, and communicate promptly with physicians and the healthcare team.
- Educate patients and families on treatment plans, medications, and preventive care strategies.
- Maintain accurate and timely documentation using Home Care Home Base (HCHB).
- Collaborate effectively with therapists, social workers, and home health aides for coordinated care.
- Participate in case conferences, staff meetings, and ongoing education.
- Adhere strictly to regulatory and agency standards for compliance and quality care.
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum of 1 year recent Home Health experience (field experience preferred).
- Proficient in using Home Care Home Base (HCHB) and OASIS assessments.
- Strong clinical assessment, critical thinking, communication, and organizational skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Able to work independently with a flexible, field-based schedule.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience managing a caseload of 2532 points weekly.
- Knowledge of Medicare and home health compliance standards.
