An opportunity is available for a dedicated and skilled Home Health Registered Nurse (RN) to provide compassionate care to patients in their homes. This 13-week contract position focuses on delivering high-quality, patient-centered care in a field-based setting, with an expectation of managing a caseload of 2532 points per week (typically 35 patient visits per day).

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
• Regularly evaluate patient progress, promptly update care teams and physicians regarding any changes in condition.
• Educate patients and families on disease processes, treatment plans, medications, and preventive strategies.
• Ensure accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary providers including therapists and home health aides to ensure seamless care coordination.
• Participate in case conferences, staff meetings, and ongoing education.
• Adhere to agency and regulatory standards for patient care and compliance.

Qualifications:

• Active RN license in the state of practice, in good standing.
• At least 1 year of recent experience in home health, with field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool is required.
• Effective clinical assessment and critical thinking skills.
• Reliability with personal transportation, valid drivers license, and auto insurance.
• Ability to manage a flexible field-based schedule independently.
• Excellent communication, time management, and organizational abilities.
• BSN preferred.
• Prior experience handling a 2532 point caseload and knowledge of Medicare/home health compliance are advantageous.
