Step into a vital role as a Home Health Registered Nurse, providing compassionate and skilled care for patients in their homes. This full-time, 13-week contract offers the opportunity to deliver high-quality, individualized nursing care while contributing to positive outcomes and patient well-being. You'll work independently across a local territory using your clinical expertise and strong interpersonal skills.

Schedule & Setting:

• Monday through Friday, 8:00 AM 5:00 PM
• Field-based, serving patients in their homes
• Caseload: 2532 points weekly (typically 35 patient visits per day)
• Electronic documentation using Home Care Home Base (HCHB)
• Conduct OASIS assessments
• Mileage reimbursement for daily travel within your assigned area

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care: wound care, medication administration, IV therapy, chronic disease management
• Monitor patient progress, identify changes, and coordinate with interdisciplinary teams
• Educate patients and families on treatment plans, medications, and health maintenance
• Document care accurately and promptly in HCHB
• Participate in collaborative team meetings, case conferences, and ongoing education
• Ensure all care meets regulatory and agency standards

Qualifications:

• Active, unencumbered RN license
• At least one year of recent home health nursing experience
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Excellent clinical judgment, organization, and communication skills
• Reliable vehicle, valid drivers license, and auto insurance
• Ability to work independently in the field
• BSN and previous experience managing full caseloads preferred
• Knowledge of Medicare and home health regulations desirable

$1,954 - $2,067/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
