Were seeking an experienced and compassionate Home Health RN for a 13-week contract opportunity. Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
• Evaluate patient progress, identify changes in condition, and communicate promptly with physicians and the care team.
• Educate patients and families about disease management, medications, and preventive care.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate effectively with interdisciplinary healthcare professionals.
• Participate in case conferences, staff meetings, and ongoing education.
• Adhere to all regulatory and best-practice standards for home health.

Qualifications:

• Active RN license in good standing for the state of practice.
• At least 1 year of recent home health RN experience (field preferred).
• Proficiency in Home Care Home Base (HCHB) and OASIS required.
• Reliable transportation, drivers license, and auto insurance.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and adapt to a flexible field-based schedule.
• BSN and prior experience managing 25-32 points per week preferred.
• Familiarity with Medicare and home health regulations is a plus.
