Job Description

Experienced and compassionate RNs are invited to make a meaningful difference in patients lives with a dynamic Home Health opportunity. Deliver high-quality, patient-centered care as you visit individuals in their homesempowering recovery, supporting independence, and providing the skilled nursing expertise they need.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • 13-week contract
  • Caseload expectation: 2532 points per week (typically 35 patient visits daily)
  • Field-based role with daily travel (mileage reimbursement provided)

Responsibilities:

  • Complete comprehensive OASIS assessments and design individualized care plans.
  • Deliver skilled interventions: wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient status, identify changes, and communicate promptly with care teams and physicians.
  • Educate patients and caregivers on diagnoses, treatments, medications, and healthy practices.
  • Ensure meticulous and timely documentation in Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary teamsincluding PT, OT, MSW, and HHAsto achieve optimal outcomes.
  • Participate in case conferences, staff meetings, and continuing education to stay current on clinical best practices.
  • Maintain compliance with all home health regulations and agency standards.

Qualifications:

  • Active RN license in good standing for the state
  • Minimum one year of recent field-based Home Health RN experience
  • Proficient with OASIS assessments
  • Direct experience using Home Care Home Base (HCHB)
  • Strong clinical assessment and independent decision-making skills
  • Reliable transportation, valid drivers license, and auto insurance
  • Exceptional communication, organizational, and time management abilities
  • BSN and experience managing 2532 point caseloads preferred
