Seeking a dedicated and compassionate Registered Nurse to deliver high-quality care in a home health setting. This opportunity allows you to make a meaningful impact on patients lives by supporting their recovery and independence in the comfort of their own homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract, full-time
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Daily travel to patient homes within the assigned territory (mileage reimbursement included)

Qualifications:

• Active RN license in good standing for the state of practice
• Minimum 1 year recent Home Health nursing experience (field experience preferred)
• Proficient in Home Care Home Base (HCHB) electronic documentation
• OASIS assessment experience is required
• Reliable transportation, valid drivers license, and current auto insurance
• Skilled in clinical assessments and critical thinking
• Strong communication, time management, and organizational abilities
• Ability to work independently and manage a flexible, field-based schedule

Preferred Experience:

• BSN degree
• Prior experience managing 2532 points per week caseload
• Familiarity with Medicare and home health compliance standards

Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing care, including medication management, wound care, IV therapy, and chronic disease management
• Educate patients and families on treatment plans, medications, and preventive care
• Maintain accurate documentation in Home Care Home Base (HCHB)
• Collaborate with a multidisciplinary team for coordinated care and participate in ongoing education and meetings
• Uphold regulatory and agency standards for patient care and documentation
