Make a direct impact in patient care as an experienced Home Health Registered Nurse providing essential care in a field-based role. This full-time, 13-week contract offers the opportunity to deliver skilled nursing services to patients in their homes, allowing for a more personal, patient-centered approach and meaningful nurse-patient relationships.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN license (in good standing)
• At least 1 year of recent Home Health nursing experience
• Proficiency with Home Care Home Base (HCHB)
• OASIS assessment experience required
• Robust clinical assessment, critical thinking, and time management skills
• Reliable transportation, current drivers license, and auto insurance
• Ability to work independently and comfortably travel daily to patient homes

Preferred:

• BSN
• Experience managing a caseload of 2532 points per week
• Familiarity with Medicare and home health compliance

Responsibilities:

• Conduct comprehensive OASIS assessments and create individualized care plans
• Deliver skilled nursing services, including wound care, IV therapy, and chronic disease management
• Maintain precise electronic documentation within HCHB
• Monitor patient progress, promptly communicate changes, and coordinate with interdisciplinary teams
• Educate patients and families on disease processes, treatments, and medications
• Participate in case conferences, staff meetings, and ongoing training to ensure quality outcomes
• Adhere to all agency and regulatory standards in patient care delivery

$1,709 - $1,807/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
