Step into a vital nursing role serving patients in their homes as a Home Health Registered Nurse (RN) on a full-time, 13-week contract. This field-based position centers on providing high-quality, patient-focused care and empowering individuals to manage their health with dignity and confidence.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Expect a caseload of 2532 points weekly (typically 35 visits daily)
- Daily travel within your assigned territory (mileage reimbursement provided)
Key Qualifications:
- Active, unencumbered RN license in the state of practice
- Minimum of one year recent field-based Home Health RN experience
- Proficient with Home Care Home Base (HCHB) and OASIS assessments
- Strong assessment, communication, and critical thinking abilities
- Reliable transportation, valid drivers license, and current auto insurance
- Comfortable working independently and managing a flexible schedule
- BSN and knowledge of Medicare/home health compliance standards preferred
Primary Duties:
- Conduct comprehensive OASIS assessments and create individualized care plans
- Deliver skilled nursing interventions (wound care, IV therapy, medication administration, chronic disease management)
- Monitor changes in patient condition and coordinate care with interdisciplinary teams
- Educate patients and families on care plans and preventive strategies
- Ensure accurate and timely documentation in HCHB
- Collaborate with therapists, social workers, and home health aides to ensure high-quality, coordinated care
- Participate in case conferences and staff meetings as required
