An opportunity is available for an experienced and compassionate Home Health Registered Nurse (RN) skilled in providing nursing care to patients in their homes. This role emphasizes delivering quality, patient-centered care, ensuring continuity and positive health outcomes for individuals in their community settings.
This full-time contract position spans 13 weeks, with a Monday through Friday schedule from 8:00 AM to 5:00 PM. The caseload includes managing 2532 points weekly, usually encompassing 35 patient visits per day across assigned territories. Daily travel is required, with mileage reimbursement provided. The position is field-based and involves using electronic documentation via Home Care Home Base (HCHB) and conducting assessments using OASIS tools.
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Administer skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate effectively with physicians and care teams.
- Provide education to patients and families on treatment plans, medications, and preventive care.
- Maintain accurate and timely documentation in HCHB.
- Collaborate with interdisciplinary teams including physical therapists, occupational therapists, social workers, and home health aides.
- Participate in case conferences, staff meetings, and ongoing education.
- Adhere to regulatory requirements and agency standards for documentation and care delivery.
Qualifications:
- Active RN license in good standing within the state of practice.
- Minimum one year of recent home health experience; field experience preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS required.
- Strong clinical assessment and critical thinking skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently and manage a flexible schedule.
- Excellent communication, time management, and organizational abilities.
Preferred:
- BSN degree.
- Experience managing 2532 points per week.
- Knowledge of Medicare and home health compliance standards.
