An outstanding opportunity awaits a skilled and compassionate Registered Nurse passionate about providing high-quality care in the home health setting. This contract position focuses on delivering patient-centered nursing services directly in patients homes, ensuring continuity of care, safety, and exceptional outcomes.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete thorough OASIS assessments and develop customized care plans tailored to each patient's needs.
• Administer skilled nursing care: wound care, IV therapy, medication administration, and chronic disease management.
• Evaluate patient progress, document changes in condition, and communicate critical updates to physicians and interdisciplinary teams.
• Provide patient and family education, supporting understanding of disease management and treatment routines.
• Accurately maintain timely electronic documentation using Home Care Home Base (HCHB).
• Coordinate care with physical therapists, occupational therapists, social workers, and home health aides.
• Engage in interdisciplinary meetings, staff training, and continuing education.
• Follow all regulatory guidelines and ensure compliance with agency and Medicare standards.

Qualifications:

• Current, active RN license in good standing.
• At least one year of recent field experience in Home Health nursing.
• Strong working knowledge of Home Care Home Base (HCHB) and OASIS.
• Demonstrated clinical assessment, decision-making, and organizational skills.
• Ability to thrive independently with a flexible, self-managed schedule.
• Reliable vehicle, valid drivers license, and current auto insurance.
• BSN preferred as well as experience with managing higher caseloads (2532 points/week).

$1,770 - $1,873/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
