This full-time, 13-week contract offers autonomy, a collaborative environment, and the chance to promote positive patient outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload of 2532 points weekly (approximately 35 patient visits per day)
• Field-based travel within an assigned territory; daily mileage reimbursed

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop tailored care plans
• Deliver skilled nursing services such as wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, identify changes, and communicate promptly with care teams and physicians
• Educate patients and families about treatment regimens, disease management, and medications
• Accurately complete electronic documentation using Home Care Home Base (HCHB)
• Cooperate with physical/occupational therapists, medical social workers, and home health aides for a holistic approach to care
• Attend case conferences, team meetings, and continuing education as needed
• Ensure adherence to all documentation, regulatory, and care delivery standards

Qualifications:

• Active and unencumbered RN license in state of practice
• Minimum one year of recent field-based home health RN experience
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Strong assessment, organizational, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and effectively manage a flexible schedule
• BSN and experience with Medicare standards strongly preferred

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
