This full-time, 13-week contract position offers an outstanding opportunity for an experienced Home Health RN to showcase expertise in a supportive and dynamic field environment.

Schedule & Setting:

• Monday through Friday, 8:00 AM 5:00 PM
• Field-based: daily visits to patient homes within a designated territory (mileage reimbursement provided)
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Electronic documentation using Home Care Home Base (HCHB)
• OASIS assessment tool

Core Responsibilities:

• Complete comprehensive OASIS assessments and design individualized care plans
• Deliver skilled nursing services: wound care, IV therapy, medication administration, and chronic disease management
• Continuously evaluate health status, report changes, and ensure care team coordination
• Educate patients and families on conditions, treatments, and preventive practices
• Maintain timely and accurate clinical documentation
• Collaborate with interdisciplinary team members, including therapists and social workers
• Participate in case conferences, staff meetings, and ongoing professional development
• Strictly adhere to all regulatory protocols and best practices

Qualifications:

• Active, unencumbered RN license in state of practice
• At least 1 year of recent home health clinical experience (field-based strongly preferred)
• Direct experience with HCHB and OASIS is mandatory
• Skilled in independent clinical decision-making, patient assessment, and effective communication
• Reliable transportation, valid drivers license, and current auto insurance required
• Strong time management and organizational skills
• BSN and familiarity with Medicare and compliance standards are preferred

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
