Job Description

An opportunity is available for a skilled Home Health Registered Nurse (RN) to provide expert nursing care to patients in their homes. This role emphasizes delivering thorough, patient-focused care, requiring strong clinical skills and the ability to work independently across a designated territory.

Responsibilities:

  • Perform comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify changes in condition, and communicate promptly with healthcare teams and physicians.
  • Educate patients and families regarding disease processes, treatment plans, medications, and preventive care.
  • Maintain accurate and timely documentation using Home Care Home Base (HCHB).
  • Coordinate care with interdisciplinary team members such as physical therapists, occupational therapists, medical social workers, and home health aides.
  • Participate in case conferences, staff meetings, and continuing education activities.
  • Adhere to all regulatory guidelines and agency standards related to patient care and documentation.

Qualifications:

  • Active RN license in good standing for the state of practice.
  • Minimum of 1 year recent home health experience, preferably field-based.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
  • Strong clinical assessment and critical thinking abilities.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Excellent communication, organizational, and time management skills.
  • Ability to work independently with a flexible field schedule. Preferred:
  • Bachelor of Science in Nursing (BSN).
  • Experience managing caseloads of 2532 points per week.
  • Knowledge of Medicare and home health compliance standards.

Schedule and Location:

  • Full-time, 13-week contract.
  • Monday through Friday, 8:00 AM 5:00 PM.
  • Caseload averaging 35 patient visits per day (2532 points weekly).
  • Field-based position requiring daily travel to patient homes within an assigned territory; mileage reimbursement provided.
  • Documentation completed electronically via HCHB.
