Join a dynamic care team dedicated to providing expert nursing services to patients in their homes. This contract opportunity is ideal for a compassionate Registered Nurse specializing in home health and committed to delivering high-quality, patient-centered care. Youll operate independently, using strong organizational and clinical skills while leveraging your expertise in OASIS and Home Care Home Base (HCHB).

Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Field-based position, daily travel required within assigned territory
• Caseload: 2532 points per week (typically 35 patient visits per day)

Essential Duties:

• Complete comprehensive OASIS assessments and establish individualized care plans.
• Deliver skilled nursing care, including wound management, IV therapy, chronic disease management, and medication administration.
• Continuously evaluate patient needs, monitor progress, and promptly communicate changes to the care team.
• Educate patients and families about diagnoses, treatment regimens, and prevention strategies.
• Maintain detailed electronic documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to ensure coordinated and effective patient care.
• Participate in team meetings, case conferences, and professional development activities.
• Uphold compliance with regulatory and agency protocols for care delivery and documentation.

Qualifications:

• Active and unrestricted RN license in state of practice required.
• At least 1 year of recent home health nursing experience; field experience highly preferred.
• Proficiency in HCHB and experience with OASIS documentation mandatory.
• Strong assessment, communication, and organizational abilities.
• Reliable transportation, current drivers license, and valid auto insurance needed.
• BSN and Medicare compliance knowledge preferred.
