An exciting opportunity is available for an experienced and compassionate Home Health Registered Nurse. In this contract position, you'll deliver skilled nursing care directly to patients in their homes, focusing on positive outcomes and quality patient-centered care. You'll thrive in an autonomous field-based role that emphasizes clinical excellence, transparent communication, and efficient collaboration with interdisciplinary teams.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and formulate individualized care plans tailored to patient needs
- Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress, document changes, and promptly communicate updates to healthcare teams and physicians
- Provide thorough education to patients and families on disease management, treatment protocols, and preventive care
- Maintain accurate and timely documentation using Home Care Home Base (HCHB)
- Collaborate effectively with therapists, social workers, and home health aides to ensure cohesive care delivery
- Attend case conferences, staff meetings, and participate in ongoing professional education
- Uphold both regulatory and agency standards in all aspects of patient care
Qualifications:
- Active RN license in good standing
- Minimum of one year of recent home health nursing experience (field experience preferred)
- Proficient in Home Care Home Base (HCHB) and OASIS documentation
- Strong clinical assessment, critical thinking, and organizational skills
- Reliable transportation, current drivers license, and valid auto insurance
- Ability to work independently and manage a dynamic, field-based caseload
- BSN and experience managing 2532 points per week preferred
- Familiarity with Medicare home health compliance standards is a plus
