An opportunity is available for a dedicated RN with a background in home health nursing to provide expert care to patients in a field-based position. This full-time, 13-week contract involves daily visits to patient homes and the chance to make a significant, positive impact on patients lives by delivering individualized nursing care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Expectation: 2532 points per week (typically 35 patient visits per day)

Responsibilities:

• Complete thorough OASIS assessments and develop personalized care plans for each patient.
• Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patients for changes in condition, update the care team and physicians as needed, and modify plans accordingly.
• Educate patients and families on diagnoses, care plans, medications, and preventative strategies.
• Meticulously document all care activities in the Home Care Home Base (HCHB) system.
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides to ensure high-quality, coordinated care.
• Participate in interdisciplinary meetings and ongoing education sessions.
• Adhere to compliance guidelines and agency protocols throughout the patient care process.

Qualifications:

• Active RN license (in good standing) in the state of practice
• At least 1 year of recent home health experience (field experience highly preferred)
• Proficient in Home Care Home Base (HCHB) and OASIS documentation
• Demonstrated clinical assessment and critical thinking ability
• Dependable transportation, current drivers license, and auto insurance
• Ability to manage a mobile work schedule and work independently
• Effective written and verbal communication skills

Preferred Qualifications:

• BSN degree
• Experience managing a 2532 point caseload
• Familiarity with Medicare and home health compliance
