Job Description
Deliver compassionate, high-quality care as a skilled Registered Nurse specializing in home health. This contract opportunity requires an experienced RN adept at providing patient-centered nursing in a field-based setting. The ideal candidate is organized, proactive, and comfortable working autonomously while maintaining exceptional standards of clinical care and patient interaction.
Schedule & Setting:
- Full-time, 13-week contract
- Monday through Friday, 8:00 AM 5:00 PM
- 2532 points per week (typically 35 patient visits per day)
- Daily travel to patient homes within an assigned territory
Essential Qualifications:
- Current, active RN license (in good standing) in the state of practice
- At least 1 year of recent Home Health nursing experience (field experience preferred)
- Proficient use of the Home Care Home Base (HCHB) system
- OASIS assessment experience required
- Solid clinical assessment and independent critical thinking skills
- Reliable transportation, valid drivers license, and proof of auto insurance
- Strong organizational, communication, and time management abilities
Preferred:
- BSN degree
- Experience managing 2532 points per week
- Knowledgeable regarding Medicare and home health compliance standards
Core Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing services (wound care, IV therapy, medication administration, chronic disease management)
- Track patient progress and report changes to the care team and physicians
- Educate patients and families on diagnoses, treatment plans, medications, and preventive strategies
- Ensure accurate, timely documentation in HCHB
- Work collaboratively with physical and occupational therapists, social workers, and aides
- Attend case conferences, participate in ongoing education, and uphold regulatory standards
