Deliver high-impact nursing care where it matters mostas a Home Health Registered Nurse delivering essential support to patients in need. This role offers the chance to work closely with patients and their families, directly influencing outcomes and quality of life within the comfort of their homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and design personalized care plans.
• Deliver skilled nursing care, such as medication administration, wound care, IV therapy, and management of chronic conditions.
• Monitor patients progress, identify changes in health status, and quickly communicate with the healthcare team and physicians.
• Educate patients and families about diagnosis, medications, treatment options, and preventive care.
• Maintain accurate, timely documentation in Home Care Home Base (HCHB).
• Collaborate with other healthcare professionals, including therapists, social workers, and home health aides for comprehensive care.
• Participate in case conferences, staff meetings, and ongoing professional development.
• Ensure compliance with all regulatory guidelines and documentation standards.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum of 1 year recent field-based Home Health RN experience.
• Proficiency in Home Care Home Base (HCHB) and OASIS required.
• Strong assessment, communication, and critical thinking abilities.
• Reliable transportation, drivers license, and auto insurance.
• Ability to work independently and manage a flexible schedule.
• BSN preferred.
• Experience managing 2532 points per week and knowledge of Medicare home health compliance standards are a plus.
