A rewarding opportunity awaits an experienced and compassionate Registered Nurse specializing in Home Health. This position focuses on providing high-quality, patient-centered care within client residences, ensuring each patient receives comprehensive and individualized support. Candidates should demonstrate strong autonomy, organization, and the ability to collaborate with a broad care team.

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Daily field-based travel (mileage reimbursement included)

Key Responsibilities:

• Complete thorough OASIS assessments and develop tailored care plans.
• Deliver skilled nursing services, such as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient status, identify changes, and communicate promptly with the interdisciplinary team and physicians.
• Provide in-depth patient and family education regarding disease, medications, therapies, and health maintenance.
• Document all care promptly and accurately in Home Care Home Base (HCHB).
• Work closely with therapists, social workers, and aides to optimize patient outcomes.
• Participate in case conferences, staff meetings, and continuing education activities.
• Remain compliant with all agency, state, and federal documentation and care requirements.

Qualifications:

• Active RN license in the state of practice in good standing
• Minimum 1 year of recent Home Health experience (field experience preferred)
• Experience with Home Care Home Base (HCHB) and OASIS required
• Demonstrated clinical assessment and critical thinking skills
• Reliable transportation, current drivers license, and active auto insurance
• Ability to independently manage a field-based schedule with excellent time management
• Effective communication and organization abilities
• BSN preferred; experience with Medicare compliance standards and 2532 point caseloads a plus
