An opportunity is available for an experienced and compassionate Registered Nurse (RN) with a background in home health nursing. This field-based position focuses on delivering quality, patient-centered care to individuals in their homes, ensuring both independence and support throughout their healthcare journeys. This is a full-time, 13-week contract.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Duties:
- Complete thorough OASIS assessments and develop individualized care plans
- Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management
- Assess patient condition and report changes to the interdisciplinary team and physicians
- Educate patients and families about disease management, medications, and preventive care strategies
- Document care accurately and efficiently in Home Care Home Base (HCHB)
- Coordinate with physical therapists, occupational therapists, social workers, and home health aides to facilitate comprehensive care
- Participate in case conferences, meetings, and professional development as needed
- Ensure compliance with all regulatory and agency standards
Qualifications:
- Active RN license (in good standing)
- Minimum 1 year of recent home health nursing experience (field experience preferred)
- Proficient in Home Care Home Base (HCHB)
- OASIS assessment experience is required
- Strong clinical judgment and assessment skills
- Reliable transportation, valid drivers license, and auto insurance
- Self-motivated, organized, and excellent communicator
- BSN preferred
- Previous experience managing a caseload of 2532 points per week is a plus
- Knowledge of Medicare and home health compliance is beneficial

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
