Job Description
Join a dynamic healthcare team as a Home Health Registered Nurse (RN), providing essential skilled nursing care directly in patients homes. This full-time, 13-week contract role offers an opportunity to make a meaningful impact on patient outcomes through comprehensive assessments, individualized care planning, and interdisciplinary collaboration. Ideal candidates are self-driven professionals with strong clinical judgment and effective communication skills, comfortable managing a diverse caseload in a field-based setting.
Responsibilities:
- Perform comprehensive OASIS assessments and develop tailored care plans.
- Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify condition changes, and communicate promptly with physicians and care teams.
- Educate patients and families on treatment plans, disease processes, medications, and preventive care.
- Maintain accurate documentation using Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and aides to ensure coordinated patient care.
- Participate in case conferences, team meetings, and ongoing education.
- Ensure compliance with regulatory and agency standards.
Qualifications:
- Active RN license in good standing within the state.
- Minimum 1 year recent home health nursing experience; field experience preferred.
- Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessments.
- Strong clinical assessment and critical thinking abilities.
- Reliable transportation with valid drivers license and auto insurance.
- Ability to work independently and manage a flexible schedule.
- Excellent organizational and communication skills.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience managing 2532 weekly points.
- Knowledge of Medicare and home health compliance standards.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM.
- Caseload typically involves 35 patient visits daily, covering 2532 points weekly.
