Job Description
Deliver compassionate, expert care as a Hospice Registered Nurse supporting patients and families in home and residential settings. This critical role provides essential nursing care and emotional support during the end-of-life journey, focusing on comfort, dignity, and quality of life. Youll collaborate closely with an interdisciplinary team to deliver exceptional patient-centered care.
Schedule and Setting:
- Monday Friday, 8:00 AM 5:00 PM (occasional weekend/on-call rotation may be required)
- Full-time, 13-week contract
- Field-based: patient homes, assisted living, or nursing facilities
- Caseload: approximately 45 visits per day
- Daily travel with mileage reimbursement
Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans
- Deliver skilled nursing care emphasizing symptom management, pain control, and patient comfort
- Administer medications and treatments per physician orders and hospice protocols
- Monitor, document, and communicate changes in patient conditions using Home Care Home Base (HCHB)
- Educate patients, families, and caregivers on disease progression, comfort measures, and support resources
- Collaborate with social workers, chaplains, aides, and volunteers to address holistic needs
- Participate in case conferences, team meetings, and quality improvement activities
- Ensure compliance with all applicable hospice policies and regulatory standards
Qualifications:
- Active RN license in good standing
- At least 1 year recent hospice experience
- Proficiency with Home Care Home Base (HCHB)
- Strong clinical assessment and communication abilities
- Reliable transportation, valid drivers license, and auto insurance
- Ability to manage a field-based schedule independently
- Compassionate and emotionally resilient approach
Preferred:
- BSN degree
- Experience in palliative/end-of-life care
- Familiarity with Medicare hospice documentation
