We are seeking an experienced Registered Nurse specializing in home health to manage field-based patient care as part of a 13-week contract. Your attention to detail, self-direction, and commitment to clinical excellence will help deliver exceptional care outcomes and support patients independence at home.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Key Responsibilities:
- Conduct thorough OASIS assessments and build individualized care plans
- Deliver skilled nursing care (wound care, IV therapy, medication administration, chronic disease management)
- Monitor patient progress, report changes, and collaborate with healthcare providers
- Educate patients and families about disease management, treatment options, and preventive care
- Maintain timely documentation in the Home Care Home Base (HCHB) system
- Coordinate care with interdisciplinary team members including therapy, social work, and home health aides
- Attend case conferences, staff meetings, and required continuing education
Required Qualifications:
- Current, active RN license in good standing
- Minimum one year of recent home health nursing experience (field experience strongly preferred)
- Fluency with Home Care Home Base (HCHB) and OASIS
- Solid assessment, clinical, and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Excellent communication, organization, and self-management abilities
Preferred Qualifications:
- BSN
- Experience with a 2532 weekly point caseload
- Familiarity with Medicare and compliance standards
